1.Background and Objectives: Allergic rhinitis (AR) is a widespread condition, with 30 to 40% of the population experiencing symptoms during the spring season. These symptoms include rhinorrhea, nasal congestion, sneezing, and eye irritation. The Clinic began using the traditional laser for treating AR in 1993, and in 2005, switched to a diode laser-pumped Nd: GaAIAS laser. In 2010, a new 810 nm diode laser was introduced. This retrospective study evaluates the effectiveness of this laser treatment for allergic rhinitis during the 2018 season, comparing the results with a previous 2011 study. The goal was to assess the extent of improvement in each symptom and the overall efficacy of diode laser therapy.

2.Subjects and Methods: From January 8 to April 30, 2018, a significant number of patients sought treatment for seasonal allergic rhinitis at our clinic. Each patient underwent blood tests, and antigen-specific serum IgE antibody levels were measured to confirm a diagnosis of cedar pollinosis. A total of 211 patients were included in the study. The average age of the participants was 36.3 years, with 134 males and 77 females. The treatment involved lower nasal turbinate mucosal irradiation using a diode laser , delivering 810 nm at 7.5 W, with a total energy dose of 240 J/cm². A five-step evaluation system, as per the Japanese Guidelines for Allergic Rhinitis (2014), was used to assess the severity of rhinorrhea, sneezing, and nasal obstruction. The degree of improvement for each symptom was evaluated from baseline to one month after treatment. Additionally, patient satisfaction and the impact on quality of life (QOL) were also measured.

3.Results: Among the 211 patients diagnosed with cedar pollinosis via IgE antibody testing, the outcomes by month and symptom improvement were as follows:

January: 18 patients -33.4% showed improvement, 44.4% had no change, and 22.2% worsened.

February: 29 patients -10.4% showed improvement, 44.4% had no change, and 22.2% worsened.

March: 146 patients -60.3% showed improvement, 31.5% had no change, and 8.2% worsened.

April: 18 patients -77.8% showed improvement, 16.7% had no change, and 5.5% worsened.

Patient QOL was reported as follows:

January: 16.7% showed improvement, 44.4% had no change, and 38.9% worsened.

February: 17.3% showed improvement, 13.8% had no change, and 68.9% worsened.

March: 61.6% showed improvement, 29.5% had no change, and 8.9% worsened.

April: 94.4% showed improvement, 0.0% had no change, and 5.6% worsened.

The prevention of exacerbation vs. worsening for each month was as follows:

January: 77.8% prevented exacerbation, 22.2% worsened.

February: 41.4% prevented exacerbation, 58.6% worsened.

March: 91.8% prevented exacerbation, 8.2% worsened.

April: 94.4% prevented exacerbation, 5.6% worsened.

The overall efficacy rate for the trial was 52.6%, which closely matches the 53.4% efficacy rate from the 2011 study.

4.Conclusions: The 810 nm diode laser demonstrated a safe and effective treatment for the symptoms of allergic rhinitis, providing significant relief during the cedar pollen season in Japan. Additionally, patients with class 6 cedar pollinosis, based on specific IgE antibody testing, showed a higher tendency for efficacy.
