1.Low-Level Laser Therapy (LLLT) causes vasodilation, localized blood circulation and relaxation of vascularized soft muscle. Vasodilation also improves blood flow and promotes the migration of immune cells to the tissues, thus accelerating tissue repair.

BACKGROUND: Burning mouth syndrome (BMS) is a complex chronic pain disorder that severely affects patients' quality of life. Low-level laser therapy (LLLT) utilizes infrared or near-infrared light to produce analgesic, anti-inflammatory, and biostimulatory effects. The aim of this systematic review was to assess the effects of LLLT on burning pain, quality of life, and negative mood in patients with BMS.
Methods: PubMed, Embase, Cumulative Index to Nursing and Allied Health Literature (CINAHL), Cochrane Library, Web of Science and Scopus databases were searched for relevant articles up to January 2023. All randomized controlled trials published in English exploring low-intensity laser therapy (LLLT) for BMS were included in this study. The methodological quality of the included trials was assessed using the Cochrane Randomized Controlled Trials (RCT) risk of bias tool. A meta-analysis was used to assess burning pain, quality of life and negative emotions. In addition, sensitivity analyses, subgroup analyses and funnel plot analyses were performed.
Results: Fourteen RCTs with a total of 550 patients with BMS were included and met the inclusion criteria. Results showed that LLLT (assessed by the Visual Analog Scale; SMD: -0.87, 95% CI: -1.29 to -0.45, P < 0.001) was more effective than placebo LLLT or clonazepam in reducing burning pain.LLLT improved quality of life (assessed by the Oral Health Impact Scale-14; SMD: 0.01, 95% CI: -0.58 to 0.60, P = 0.97) and negative mood (assessed by the Hospital Anxiety and Depression Scale; SMD: -0.12, 95% CI: -0.54 to 0.30, P = 0.59), but none of these effects were statistically significant.
CONCLUSIONS: The meta-analysis showed that low-intensity laser therapy (LLLT) may be an effective treatment to improve burning pain in patients with BMS and positively affect quality of life and negative mood. Long-term interventions, larger sample sizes, and multidisciplinary intervention designs are urgently needed for future studies.
