BACKGROUND:The pulsed-dose oxygen-conserving device (PDOCD) is widely accepted as an efficient means to minimize the cost and inconvenience of portable oxygen therapy. While extensively used by patients during waking hours and mobility, few studies have investigated its effectiveness during sleep. This study aimed to compare heart rate and oxygen saturation (SpO2) during sleep between patients using a pulsed-dose oxygen-conserving device and those using continuous-flow oxygen.



METHODS:We examined 10 home-oxygen patients using various continuous-flow oxygen systems and prescriptions. Baseline asleep and awake SpO2 and heart rate were recorded while patients used their existing home-oxygen setups (liquid oxygen or oxygen concentrator with nasal cannula) and continuous-flow oxygen prescriptions. Subsequently, patients switched to a nasal cannula connected to a PDOCD, which was adjusted to match the awake SpO2 baseline of continuous-flow. This setting was maintained during sleep. Mean SpO2 and heart rate values, along with hours of sleep, were calculated using the oximeter software. Paired Student's t-test was employed to compare SpO2 and heart rate values.



RESULTS:A statistically significant but clinically minor difference in SpO2 was observed between continuous-flow oxygen and PDOCD use (95.7% vs. 93.2%, p = 0.043). No significant difference in heart rate was found (77.3 beats/min vs. 77.9 beats/min, p = 0.70). The sample size was sufficient to detect a heart rate difference of 5 beats/min with 80% power. For patients whose PDOCD triggering sensitivity was set to sensitive, a statistically significant but clinically minor difference in SpO2 was noted (95.6% continuous-flow vs. 93.2% PDOCD, p = 0.044). Other comparisons showed no differences, though sample sizes were too small for definitive conclusions. One patient experienced an 11% drop in SpO2 with the PDOCD due to insufficient triggering sensitivity.



CONCLUSIONS: The studied PDOCD model effectively delivered comparable oxygen therapy to continuous-flow oxygen in 9 out of 10 patients. Setting the PDOCD to the resting daytime SpO2 on continuous-flow ensures adequate oxygenation during sleep. This specific PDOCD model maintains adequate SpO2 during sleep for selected patients. However, variations in design, triggering sensitivity, and oxygen-pulse volume mean these results cannot be generalized to all patients or PDOCDs. Further research is needed to evaluate the broader performance of PDOCDs in larger populations and those with sleep-disordered breathing.
