The objective of this investigation was to assess pulpal reactions in sound human teeth to photo-biomodulation therapy (PBMT) using a 904-nm GaAs diode laser. Employing a double-blind split-mouth setup, a maxillary first premolar was randomly designated as a sham-irradiated control tooth, while its contralateral counterpart underwent active laser treatment. Two identical laser probes (COZING, COZING MEDICAL Ltd., China), distinguished only by code, were utilized for delivering sham (placebo) and laser radiation, with both operator and patient unaware of probe identity. Tooth selection for sham or laser irradiation was randomized for each treatment. Pulpal responses were evaluated via electric pulp testing (EPT), conducted 2 minutes pre-exposure and immediately post-laser irradiation (60 seconds, 30 mW average power, 25 Hz pulse frequency, 3.6 J/cm2). Treatment effects were statistically analyzed using the Wilcoxon-signed rank test. Thirty subjects provided written informed consent. The majority of participants (66.7%) exhibited analgesic effects post-PBMT (elevated EPT scores); nevertheless, nine participants (30%) reported lower EPT scores compared to the control. Both stimulatory and analgesic treatment effects were significant relative to the placebo.
